The medications Clonazepam, Quetiapine, Duloxetine are all pretty complex action, side effects and potential interactions. You have to work with a psychiatrist - your choice is which psychiatrist. But I don't think it would be unusual for certain side effects or even effects to emerge over time.
In one study the Quetiapine seemed to worsen dementia symptoms Quetiapine and rivastigmine and cognitive decline in Alzheimer's disease: randomised double blind placebo controlled trial but the mechanism isn't clear.
I would recommend you do everything you can on the less medical side of things as long as they don't hurt / interfere, while you work out with a psychiatrist how you can adjust meds. One general theory is that problems with memory might be related to inflammation, in this theory:
Music, listening or playing, singing seems to help
Meditation is just so healthy everyone should (I wish I did more!)
Exercise, short, intense, seems to induce neurogenesis
Motion in space, walking, seems to also induce neurogenesis
Diet with no sugars, (cases of dementia patients sent home, living independently again after diet change)
Sleep (its really critical but not induced by meds, except melatonin but check with psychiatrist)
Longvida Curcumin supplement but check with psychiatrist if its ok
I would actually avoid focused "brain training" with a specialised exercise because it might overwork a particular circuit. This is my personal pseudoscience, but if music works which is a diffuse mental process, conversely a focused process might not be good.
Every case is individual. If you are suffering cognitive deficits, it is hard to be the advocate you need to be to get the treatment you need. I attend a local support group for people with Mild TBI. Call the nearest brain injury association and find someone to talk to about your particular situation, whether it is a group or experienced health care practitioner.
